Propofol is the preferred induction agent for neurosurgical patients. Balanced anesthesia using propofol-fentanyl is a safer and more predictable alternative to a propofol-nitrous oxide combination. Unfortunately, many centers are unable to use propofol-fentanyl anesthesia because the dose of propofol to achieve anesthesia is too high. The purpose of this study was to determine whether a propofol-fentanyl combination would require the same dose of propofol to achieve anesthesia as a propofol-nitrous oxide combination. A randomized, double-blinded, controlled clinical study was performed. Three groups of 30 patients each were treated with one of three doses of propofol-fentanyl anesthesia: 2 mg/kg, 4 mg/kg, or 6 mg/kg of propofol-fentanyl. After induction, patients were assigned to one of three groups, one group to each dose of propofol-fentanyl, until a trial of wakefulness had been obtained. Induction doses of propofol-fentanyl were titrated to effect. Responses to verbal commands were assessed. Episodes of desaturation were also recorded. Statistical analysis was performed using analysis of variance and a Bonferroni correction for the three comparisons. Data are presented as mean +/- standard error of the mean. ANOVA revealed no difference in the time from loss of eyelash reflex to attainment of bispectral index score of less than 50 (Kruskal-Wallis test, P = 0.21) between the three groups. The groups did not differ in heart rate, mean arterial blood pressure, or use of atropine between induction and awakening. The groups also did not differ in total anesthetic dose requirements, total anesthesia time, or the number of observations of respiratory depression. The percentage of patients observed to have a respiratory rate of 20 or less was significantly less in the 6 mg/kg than in the 4 mg/kg group (P = 0.016).
